**Job Role:** Community Care Specialist

The role of a Community Care Specialist plays a vital part in the Support at Home Program, ensuring that older Australians receive high-quality care and support tailored to their individual needs. This position requires delivering person-centred care planning, coordinating services, and building strong relationships with participants and their support networks.

To achieve this goal, the Community Care Specialist manages a caseload of participants within the Support at Home Program. The role involves coordinating and overseeing the delivery of care services, working closely with participants, their families, service providers, and other health professionals to ensure care plans are tailored to each person's unique needs and preferences.

**Key Qualifications & Skills:

  • Hold a Certificate IV in Aged Care or a relevant qualification in Nursing, Allied Health, Social Work, or a related field.
  • Possess a degree or equivalent experience in aged care is highly preferred.
  • Have demonstrated experience in case management or service coordination, especially within aged care or community care sectors.
  • Be able to work effectively as part of a multidisciplinary team and communicate clearly and respectfully with families, participants, and service providers.

**Required Certifications & Documents:

  • Current Australian Driver's License.
  • National Police Check and Working with Children Check (or willingness to obtain).
  • First Aid and CPR certification.
  • A commitment to continuous professional development and training.

  • Strong analytical and problem-solving skills with the ability to assess situations, identify solutions, and make sound decisions in a timely manner.
