Job Overview

This role involves working as a Community Care Professional, providing person-centred support to clients under the Commonwealth Home Support Program (CHSP) and the Home and Community Care Program for Younger People (HACC-PYP).

The Opportunity
  • Promote independence and reablement by supporting clients in maintaining and building daily living skills.
  • Enhance health, wellbeing, and social connections by enabling participation in community life and reducing social isolation.
  • Provide flexible, person-centred care by aligning services with each client's goals, preferences, and cultural needs.
  • Support carers and families by ensuring sustainable care arrangements through respite and tailored assistance.
Key Responsibilities
  • Domestic Assistance: Support clients with household tasks such as cleaning, laundry, mopping, vacuuming, shopping, and light meal preparation.
  • Personal Care: Assist with showering, bathing, dressing, grooming, medication prompting, and general hygiene needs.
  • Individual Social Support: Provide companionship, assist with shopping, transport to essential appointments, and help clients remain connected to their community.
  • Respite Care: Provide in-home or community-based respite for carers while ensuring clients remain safe and engaged.
  • Wellbeing Monitoring: Observe and report changes in clients' health and wellbeing to support independence and safety.
  • Team Contribution: Collaborate with colleagues, attend staff meetings, and participate in training and development.
  • Administration: Maintain accurate and timely records to ensure quality service delivery and compliance.

Candidates should possess a genuine commitment to person-centred care, be physically fit, and have knowledge of CHSP and HACC-PYP principles.

About You
  • A commitment to person-centred care and supporting older people and younger people with disabilities.
  • Physically fit and able to perform everyday tasks such as vacuuming, lifting, and practical household support.
  • Knowledge of CHSP and HACC-PYP principles, including independence, wellbeing, social inclusion, and carer support.
  • Current valid Victorian Driver's licence and access to a safe and appropriately registered and comprehensively insured vehicle suitable for transporting clients.
  • Must hold or be able to obtain an Employee Working with Children Check.
