Care Management Specialist

We are seeking a dedicated and compassionate Care Management Specialist to join our team.

Job Description:

The successful candidate will provide casework and case management services for Aboriginal children in out-of-home care, employing a trauma-informed approach. This involves conducting visits, supporting foster carers, and implementing Cultural Support Plans that promote cultural identity and connection.

Your Key Responsibilities:

  • Provide holistic support to children and their families, ensuring their safety and well-being.

  • Develop and implement individualised plans that cater to the unique needs of each child.

  • Collaborate with foster carers, family members, and other professionals to achieve positive outcomes for children.

  • Maintain accurate and timely records of client interactions and progress.

About You:

  • You possess a strong commitment to working with Aboriginal communities and an understanding of their values and cultures.

  • Previous experience working with children and families in a case management or social work context is essential.

  • You are able to work collaboratively as part of a team and communicate effectively with stakeholders.

  • A valid Police Check and Working with Children's Check are mandatory prerequisites.
