**Job Title: Client Liaison Officer**

We are seeking a skilled professional to undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with clients to assist them in achieving their identified goals.

The role involves working effectively with clients to engage them and provide support and guidance on budget management and financial sustainability, as well as advocating strongly for your clients.

To be successful in this role, you will require:

  • A Certificate IV in Health, Community Services, Aged Care or Disability Support
  • A proven understanding of assessments, packaged care and case management
  • Exceptional communication skills
