Job Title: Housing Pathways Program Case Manager
About the RoleThis is an exciting opportunity for a motivated and compassionate individual to join our team as a Housing Pathways Program Case Manager.
The successful candidate will be responsible for assisting clients who are at risk of homelessness to access stable and safe housing options. This will involve working closely with court services, community organizations, and other stakeholders to provide comprehensive support and advocacy to our clients.
We are seeking a candidate who has experience in case management, social work, or a related field. The ideal candidate will have strong communication skills, be able to work effectively in a fast-paced environment, and possess a deep understanding of the complexities of homelessness.
In this role, you will have the opportunity to make a real difference in the lives of individuals and families who are struggling to find stability and security. You will be part of a dedicated team that is passionate about providing exceptional service and making a positive impact in our community.
Key Responsibilities- Assist clients who are at risk of homelessness to access stable and safe housing options
- Work closely with court services, community organizations, and other stakeholders to provide comprehensive support and advocacy to our clients
- Develop and implement individualized case plans to address the unique needs and goals of each client
- Maintain accurate and up-to-date records of client interactions and progress
- Bachelor's degree in Social Work or a related field
- Minimum 2 years' experience in case management, social work, or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Deep understanding of the complexities of homelessness
