**Job Summary:**

The Residential In Reach service aims to reduce unnecessary presentations to the Emergency Department by providing acute support, early intervention and patient focused healthcare within Residential Aged Care Facilities.

This role provides visible and accessible clinical leadership to a team of clinical nurse consultants, with expertise in gerontology and emergency care. The role works predominantly in a supervisory capacity, with opportunity for clinical reviews to support the team and provide supervision.

**Key Responsibilities:**

  • Collaborate with key personnel to drive innovation and growth in aged care
  • Lead and manage a small team of senior nurses to achieve strategic objectives

**Essential Skills and Qualifications:**

  • Proven clinical expertise and experience working in an acute setting
  • Strong collaborative working style and ability to build effective relationships
  • Excellent communication skills and passion for continued professional development
  • AHPRA registered Nurse with a strong understanding of gerontology and emergency care principles
