Dementia Care Services Leader

We are seeking a highly skilled and compassionate individual to lead our dementia care services. As the Dementia Lead, you will play a crucial role in ensuring that our residents living with dementia receive high-quality, person-centered care.

Reporting to the Clinical Lead, you will be responsible for coordinating care services, identifying and addressing clinical, health and social care needs, and promoting independence and choice.

Key Responsibilities:
  • Coordinating care services to meet the needs of residents living with dementia
  • Leading multidisciplinary meetings and family discussions to ensure effective communication and collaboration
  • Managing ongoing reviews and assessments of residents, including liaison with allied health professionals
  • Developing and implementing quality improvement initiatives to enhance dementia care
  • Providing training and mentorship to staff to promote clinically relevant engagement and psychosocial support strategies

About You:

  • You hold a Registered Nursing qualification with current AHPRA registration
  • You have extensive experience working in dementia care as a Registered Nurse
  • You possess excellent leadership and communication skills, with the ability to train and mentor others
  • You are committed to delivering high-quality, person-centered care that promotes independence and choice
