Job Role: Care Services Coordinator

The primary objective of this position is to provide comprehensive care services to individuals who have experienced significant injuries. As a key member of the team, you will play a vital role in supporting these individuals throughout their recovery journey and eventual return to work or home.

Duties and Responsibilities:
  • Assess the needs of clients and develop personalised care plans to ensure optimal outcomes.
  • Coordinate and case manage treatment and care services, ensuring seamless delivery and effective results.
  • Oversee the rehabilitation process, guaranteeing regulatory compliance and adherence to best practices.
  • Establish strong relationships with clients and healthcare providers, fostering a collaborative environment.
Essential Qualifications:
  • Relevant degree in Physiotherapy, Occupational Therapy, or Nursing.
  • Current registration with the relevant governing body (e.g. AHPRA).
  • Proven experience in case management and patient care.
  • Relevant certifications and clearances (e.g. NDIS workers screening, police check).
