
Comprehensive Care Coordinator

We are seeking a skilled Speech Therapist to join our team of professionals dedicated to delivering exceptional care to older Australians. This is an exciting opportunity for a motivated individual to make a real difference in the lives of our clients.
This role will see you working in a diverse range of settings, including hospitals and community environments. You will have the opportunity to build strong therapeutic relationships with clients and their families, while also supporting smooth transitions of care between different healthcare settings.
To be successful in this role, you will need to possess excellent time management and organisational skills, as well as a strong commitment to continuous learning and professional development. You will also need to have a solid understanding of aged care and/or rehabilitation principles, and be able to work effectively in a collaborative team environment.
- Deliver comprehensive assessments, tailored treatments, and meaningful interventions for older clients.
- Build strong therapeutic relationships with clients and their families.
- Support smooth transitions of care between hospital and community settings.
Requirements:
- Bachelor or postgraduate degree in speech therapy providing eligibility for membership of a relevant professional association.
