A career opportunity exists for an experienced Occupational Therapist to work with older adults in a transitional aged care program. The successful candidate will be responsible for conducting home visits and supporting clients to maintain independence and quality of life.

About the Role

The ideal candidate will possess excellent communication skills, both verbally and in writing, and be able to work collaboratively as part of a multidisciplinary team. A strong understanding of gerontology and experience working with older adults is highly desirable.

The role requires the ability to conduct thorough assessments and develop effective plans to support clients achieve their goals and safely transition from hospital back into the community.

  • Occupational Therapist with current Authority to Practice with AHPRA
Key Responsibilities

The key responsibilities of this role include:

  1. Conducting home visits to assess client needs and develop effective plans
  2. Working collaboratively with a multidisciplinary team to ensure seamless care coordination
  3. Developing and maintaining relationships with clients and their families
  4. Providing education and support to clients and their families on how to maintain independence and quality of life
  5. Collaborating with other health professionals to ensure comprehensive care
