**Clinical Care Leader Role Summary:**


We are seeking an experienced Clinical Care Coordinator to join our team as a Clinical Care Leader. This pivotal role will involve leading the clinical operations of our residential aged care home, ensuring our residents receive exceptional care in line with industry standards.

As a dynamic and experienced leader, you will be responsible for facilitating case conferences, providing expert case management, and monitoring and improving clinical outcomes through audits and quality initiatives.

This is an ideal role for someone with strong leadership skills, a genuine passion for uplifting people and culture, and a commitment to best practice and continuous improvement.

**Responsibilities:**

  • Leading and coordinating clinical operations within the residential aged care home;
  • Facilitating case conferences to develop care plans;
  • Providing expert case management alongside the Residential Care Manager;
  • Monitoring and improving clinical outcomes through audits and quality initiatives;
  • Supporting policy development and implementation for resident care;
  • Coaching and inspiring staff to deliver outstanding care.

**Requirements:**

  • Current AHPRA registration;
  • Proven clinical and leadership experience in residential aged care;
  • Strong skills in assessments, care planning, and change management;
  • Sound understanding of aged care funding models;
  • Commitment to best practice and continuous improvement.
