Job Description:

We are seeking a skilled and compassionate Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will play a vital role in providing high-quality clinical care to residents in our purpose-built aged care home.

Main Responsibilities:
  • Provide clinical expertise and leadership to clinical teams across the facility.
  • Assist in leading carer teams to meet all clinical needs of residents.
  • Manage assessment, documentation, and submission of requirements within the facility.
  • Support effective documentation and full clinical team user support of policies, procedures, and IT system requirements.
  • Coordinate assessment, care planning, and review processes.
  • Support the admission of new residents for optimal care assessments and initial consultations.
  • Ensure accurate information is recorded and in accordance with organisational and legislative requirements.
  • Input and oversight into facility team rosters and ensuring optimal skill mix available for delivery of care.
  • Manage and document clinical risk within the facility.

You Will Offer

  • Empathy for residents and commitment to improving quality of care.
  • Minimum 1 year experience as a Care Coordinator or similar leadership position.
  • Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
  • Postgraduate qualifications in aged care advantageous but not essential.
  • Experience and knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
  • Engaging style to influence and develop clinical leadership skills.
  • High level of computing skills to maintain documentation and reporting requirements.
  • Confident team player with excellent communication and interpersonal skills.
  • Ability to manage multiple stakeholders and have a balanced approach to emergency situations.
  • NDIS Workers Screening Clearance or ability to apply.
