As a skilled healthcare professional, you will provide person-centred care to Clients in the community, ensuring a safe environment and applying your clinical skills to improving their quality of life through various activities.
The role requires assessment, planning, and implementation of client care within your scope, considering physical, social, and psychological requirements. You will also create and update care plans to reflect developing needs, ensure clinical quality compliance standards are met, and work collaboratively with other multi-disciplinary team members.
Key Responsibilities:- Assess and plan individualized care for Clients
- Implement care plans in accordance with established policies and procedures
- Monitor and evaluate the effectiveness of care interventions
- Collaborate with other healthcare professionals to achieve optimal Client outcomes
- Current registration as a Registered Nurse with AHPRA
- Excellent communication and clinical problem-solving skills
- Current Australian Driver's Licence (Green Provisional minimum)
- Roadworthy and insured vehicle to use for work
- Ability to work autonomously and manage time effectively
