The Targeted Care Packages (TCP) Key Worker plays a vital role in supporting young people to transition into more suitable care arrangements, where their experience of care may be more positive and their needs better met.
This may involve working with young people to develop and implement tailored care packages, which include an allocation of funding tailored to their individual needs. The TCP Key Worker is responsible for the ongoing implementation and case management of these packages, ensuring that the young person's needs are met and their progress is monitored.
Key Responsibilities- Develop and implement care packages for young people, in collaboration with other stakeholders
- Provide ongoing support and assistance to young people, to help them transition into more suitable care arrangements
- Monitor and evaluate the effectiveness of care packages, and make adjustments as needed
- Work collaboratively with other teams and stakeholders to ensure a comprehensive approach to supporting young people
- Maintain accurate records and reports, in accordance with Allambi Care's policies and procedures
- Diploma of Community Services or equivalent qualification
- Experience in case work practice with children and young people
- Leadership experience, with the ability to support and manage a team
- Autonomy and ability to work independently, with minimal supervision
- Strong knowledge of Allambi Care's policies and procedures
- Excellent communication and interpersonal skills
- Ability to work in a high-stress environment, with a focus on providing high-quality support to young people
- Full-time employment
- 4 weeks annual leave per year
- 1 roster day off per month
- Opportunity to receive fringe benefits tax (salary sacrifice) on commencement of employment
- Monday to Friday work week
