The Targeted Care Packages (TCP) Key Worker provides support and assistance to young people to enable them to transition into more suitable care arrangements where their experience of care may be more positive, and their overall needs better met.
This may include moving into independent or semi-independent living, home-based care, foster care, living with family/friends, or reunification with their parents. The packages include an allocation of funding tailored to the needs of each individual young person; the TCP Key Worker is responsible for the ongoing implementation and overarching case management responsibility of these packages.
Key Responsibilities- Provide support and assistance to young people to enable them to transition into more suitable care arrangements.
- Manage the ongoing implementation and case management responsibility of Targeted Care Packages.
- Work collaboratively with a team to share information, be receptive to the ideas of others, and work within team relationships and dynamics in a professional manner.
- Respond to a wide range of operational issues with flexibility, initiative, creativity, and innovation.
- Diploma of Community Services or equivalent Diploma level qualification.
- Completed top-up units.
- Experience within case work practice with children and young people.
- Demonstrated experience in leadership of employees.
- Demonstrated autonomy when supporting multiple teams and clients.
- Full-time employment.
- Four weeks annual leave per year.
- One roster day off per month.
- Opportunity to receive fringe benefits tax (salary sacrifice) on commencement of employment.
