Job Overview:

Delivering primary healthcare services to Aboriginal individuals, families and community groups is a core aspect of this role.

The ideal candidate must be an Aboriginal and/or Torres Strait Islander with the ability to arrange and coordinate healthcare plans for clients.

Key Responsibilities include:

  • Checking recalls required for clients using the Communicare Database
  • Performing data entry as necessary
  • Working in partnership with clients
  • Providing screening and procedures as requested by GPs
  • Conducting chronic disease care plans
  • Ensuring client accessibility to other organisations

Additional responsibilities involve providing education on disease prevention, independent living skills and respite availability to clients and carers.

The successful applicant should possess a Certificate IV or Diploma in Aboriginal Health, knowledge of current health issues impacting Aboriginal communities and understanding of public health principles including health education and promotion.
