The Clinical Case Coordinator will support the Child and Youth Assessment & Therapeutic Service by collecting background information about clients, liaising with stakeholders, and managing administrative tasks. This role aims to provide seamless assessment, treatment, management, and support access to the National Disability Insurance Scheme through a multidisciplinary team.
Responsibilities- Collection of client data and background information
- Liaison with internal and external stakeholders
- Administrative tasks as required
- Client-focused, family-centred, and trauma-informed clinical case management
- Knowledge of child development and neurodevelopmental disorders
- Minimum diploma qualification or relevant experience in a clinical coordination role
- Demonstrated experience and skills in providing clinical case management support to vulnerable families
