Job Title: Home Care Partner

Our organization seeks a skilled Home Care Partner to support individuals in their care needs, ensuring they can live safely at home.

The successful candidate will work closely with participants, their families, and healthcare professionals to provide person-centered care that meets their unique needs and goals.

The key responsibilities of this role include:

  • Conducting assessments of participants in their homes to identify their care needs and develop personalized care plans.
  • Ensuring follow-up actions are taken to reduce risks, refer allied health professionals, order equipment, and evidencing participant outcomes.
  • Ongoing case management to support participants' clinical and holistic needs, monitoring and communicating on their wellbeing and reporting progress.
  • Documenting participant goals, progress, and outcomes using person-centered, wellness, reablement, and enabling strategies.
  • Undertaking administrative duties, including data entry, processing invoices, following up participant enquiries, and ensuring documentation meets program guidelines.

The ideal candidate will possess strong administration skills, time management, prioritizing, and attention to detail skills. They should be able to work in a fast-paced environment and prioritize tasks effectively.

To succeed in this role, the candidate must have:

  • Advanced computer skills, including CRM software.
  • Strong administration skills and organizational skills.
  • Ability to multi-task and work in a fast-paced working environment.
  • Strong skills in prioritizing and problem-solving.
  • Ability to work in a wellness model – reablement and enablement.
