About the Role
As a key member of our team, you will play an integral part in delivering individualised services to customers in their homes.
This involves undertaking assessments to ensure the best possible outcomes for participants, as well as ensuring follow-up to reduce risks and referring allied health professionals when necessary.
You will be responsible for ongoing case management, supporting participants in both clinical and holistic needs. This will involve monitoring and communicating participant wellbeing and reporting on progress.
In addition, you will document participant goals, progress and outcomes using person-centred strategies that focus on wellness, reablement and enablement.
You will also be responsible for extensive administrative duties including data entry, processing invoices and documenting all interactions with participants and stakeholders.
Liaising with community-based agencies and service providers to ensure integrated and cohesive service delivery is also a key aspect of this role.
You will assist in participant service schedules and work closely with participants to manage their budgets in accordance with program guidelines.
Effective communication with key stakeholders internally and externally is crucial, as is community engagement and promoting the organisation through various channels.
Finally, you will oversee and manage participant budgets, working under the guidance of medical practitioners and Allied Health professionals as needed.
- Key Responsibilities:

- Extensive administrative duties including data entry, processing invoices and documenting all interactions

- Assist in participant service schedules and work with participants with their budgets and monitor participant's budgets in accordance with program guidelines
- Communication with key stakeholders internally and externally
- Community Engagement: engagement with key community stakeholders promoting Just Better Care
- Document all evidence requirements in line with Compliance, ensuring 100% Compliance
- Oversee and manage participant's Budgets (Govt Funding)
- Work with or under the guidance of medical practitioners and Allied Health professionals
Requirements
- Essential Skills and Qualifications:
- Advanced computer skills (CRM)
- Advanced administration skills and organisational skills
- Ability to multi-task and work in a fast-paced working environment
- Strong skills in prioritising and problem-solving
- Superior customer service skills
- Attention to detail
- Ability to work in a wellness model – reablement and enablement
- Demonstrated strengths in team work and working autonomously
- Previous experience in KPI’s and working to deadlines
- Current CPR / First Aid Certificate
- Current and satisfactory Criminal Record Check
- Current and satisfactory NDIS Screening Card and Working With Children Check
- Current Australian Driver's Licence
Desirable Qualifications:
- Previous experience in the home care package, community care or health/aged care sector
- Understanding of Home Care Package program Guidelines and Compliance or previous community nursing experience
- Minimum Bachelors in Nursing - Current AHPRA
- Registered Nurse - Current AHPRA
