Job Summary
We are seeking a skilled Enrolled Nurse to join our team as a Home Care Coordinator. The successful candidate will be responsible for delivering high-quality patient care and coordinating services for clients in the community.
About the Role
The Home Care Coordinator will work closely with clients, families, and healthcare professionals to develop personalized care plans that meet the unique needs of each individual. This role requires excellent communication and interpersonal skills, as well as the ability to work independently and make sound judgments.
Key Responsibilities
- Conduct thorough assessments and develop comprehensive care plans for clients
- Coordinate services and provide ongoing support to clients and their families
- Collaborate with healthcare professionals to ensure seamless care delivery
- Manage client budgets and prioritize services to meet individual needs
- Maintain accurate records and reports to ensure quality care and service delivery
Requirements
- Diploma of Nursing with current AHPRA registration
- Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009)
- Valid driver's license (C class)
- Experience working in a Community or Residential Aged Care setting
- Excellent communication and interpersonal skills
- Ability to work independently and make sound judgments
