Care Coordinator for Community Aged Care Services

Dementia and Aged Care Services (DACS) is a leading provider of community care services in New South Wales, dedicated to enhancing the quality of life for clients and their families.
We are seeking an experienced Full Time Aged Care Case Manager to further develop and grow our Home Care Package business in Bateman's Bay.
The successful candidate will thrive in this role, delivering high customer service excellence with attention to detail and ability to manage consumer budgets across different level packages.
Key Responsibilities
- Conduct in-home assessments, referrals, care plan reviews and goal planning for consumers in the South Coast region.
- Manage client budgets and report on financial performance.
- Utilize excellent time management skills to maintain a caseload of clients.
- Communicate effectively with clients regarding changing care needs.
- Maintain relationships with internal and external stakeholders.
- Demonstrate empathy and understanding towards the needs of older people.

Requirements for Success
- Solid case management experience is essential.
- Regular travel around the Bateman's Bay area is required.
- Knowledge and enthusiasm for community aged care services and consumer-directed care environments are necessary.
- A Diploma-level qualification or above in community, aged, health, or another discipline is desirable.
- An open driver's license and access to a reliable, registered motor vehicle with full comprehensive insurance are required.
- Strong computer skills are essential.
- Covid vaccinations, including a booster, are mandatory.
- A clear national police check upon role offer is necessary.
