Community Support Specialist

About the Role

As a Community Support Specialist, you will provide support and care to vulnerable clients within their community. You will work collaboratively with a professional and compassionate team of Care Coordinators to deliver high-quality services.

Colbrow Homecare provides premium support services for individuals requiring aged care at home, those with physical, sensory and/or intellectual disabilities, as well as those injured in road traffic accidents or the workplace.

Maintaining client privacy, earning their respect, and preserving their dignity are core values that guide our approach.

Key Responsibilities

  • Deliver personalized support services to enhance clients' independence, social and community participation, and help them achieve their goals in accordance with their care plans.
  • Support clients to maintain their independence and dignity by assisting with activities of daily living, including personal care, social support, meal preparation, exercise, community access, transport, medication monitoring, domestic assistance.

Required Skills and Qualifications

  • Certificate III/IV in Community Care, Individual Support or Disability.
  • Proven experience in this or a similar role/field.
  • Current Working with Children's Check.
  • Current NDIS Worker Screening Check (includes Police Check).
  • First Aid Certificate (HLTAID011) and CPR (HLTAID009).
  • Mandatory competencies (as required by Colbrow Homecare).
  • Valid working rights in Australia.
