Care Managers play a vital role in supporting individuals in their homes, utilizing interRAI and other computer-based tools to assess their needs. Key responsibilities include:

  • Providing expert clinical guidance and oversight to support workers, ensuring comprehensive care planning and delivery.
  • Directing and delegating tasks to support workers, ensuring safe and effective administration of medication.
  • Conducting comprehensive assessments with clients, families, and whanau, in collaboration with primary health providers.
  • Developing individual service plans, utilizing a determined tool, to ensure tailored support.
  • Coordinating clinical requirements, including client reviews, skill mix, and competencies for support workers.
  • Maintaining key relationships with stakeholders, including clients, families, whanau, and relevant agencies.
  • Delivering services aligned with contractual obligations and organisational policies and procedures.
  • Undertaking administrative tasks associated with the role.
  • Demonstrating a high level of commitment to and understanding of the organisation's Health and Safety Management System.
  • Upholding Visionwest Community Trust's Mission and Christian ethos.
  • Participating in ongoing education and professional development.
About you

Ngā uara

  • Registered Health Professional (e.g., Registered Nurse, Occupational Therapist, or Physiotherapist).
  • Current annual practicing certificate with the relevant regulatory body.
  • Current Driver's license (Full Licence Class 1).
  • Knowledge of working with older people and/or those with disabilities in the community.
  • Understanding of relevant Health & Safety and Employment legislation.
  • Excellent interpersonal skills, team building skills, analytical and problem-solving skills.
  • Competent in working with Human Services databases and intermediate-level Microsoft Word skills.
  • Knowledge of the local area.
Desirable

  • Understanding of the social model of service delivery in a community care environment.
  • Working knowledge of the Health & Disability Sector Standards and robust quality systems.
  • A knowledge of interRAI would be an advantage, but training will be provided.
