About this role
Care Managers at Visionwest Community Trust are responsible for providing expert clinical guidance and support to multidisciplinary teams. This involves using advanced assessment skills and tools to plan and oversee support for clients in their homes.
Key Responsibilities
- Acts as an expert clinical resource using advanced assessment skills and appropriate assessment tools to plan and oversee support within a multidisciplinary team.
- Directs and delegates to ensure appropriate and safe administration of medication by Support Workers.
- Provides comprehensive assessments with clients in collaboration with family/whanau and other primary health providers.
- Develops individual service plans in collaboration with clients, family, whanau, and other primary health providers.
- Coordinates clinical requirements, including clients' reviews, skill mix, and competencies for Support Workers.
- Maintains key relationships with stakeholders, including clients, families/whanau, and relevant agencies.
- Delivers services aligned with contractual obligations and Organisation's Policies and Procedures.
- Undertakes administration tasks associated with the role.
- Demonstrates a high level of commitment to and understanding of the Organisation's Health and Safety Management System.
- Upholds Visionwest Community Trust's Mission and Christian ethos.
- Participates in own ongoing education and professional development.
About you
To be successful in this role, you will need to be a Registered Health Professional with a current annual practicing certificate and a current Driver's Licence. You will also need to have knowledge of working with older people and/or those with disabilities in the community, as well as excellent interpersonal skills and analytical and problem-solving skills.
