About the Role
We are seeking a dedicated and passionate Social Worker to join our team at Grampians Health Ballarat. As a Social Worker Grade 2, you will play a vital role in enhancing the psychological and social well-being of our patients. Your key responsibilities will include conducting psychosocial and risk assessments, facilitating complex discharge planning, and providing support to both patients and their families.
Key Responsibilities
- Ensure the smooth transition of clients through the healthcare system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients/clients and their families.
- Undertake psychosocial and risk assessments (family violence, elder abuse, and child safety).
- Complex discharge planning.
- Communicate with a wide range of professionals.
- Collaborate in pursuit of patient and team needs/goals.
Skills and Experience
- Eligibility for practising membership of the Australian Association of Social Workers (must have a bachelor or qualifying master's degree).
- Demonstrated ability to monitor, evaluate, and modify own performance through reflective practice.
- Ability to adapt, be flexible, and learn quickly.
- Work well under pressure.
- Ability to plan and problem-solve.
- Demonstrated knowledge in Social Work theory and practice skills.
- Some knowledge of working in acute/sub-acute environments is desirable.
- Ability to provide Social Work supervision or work toward this skillset.
- Diverse experience: opportunities to rotate caseloads, gain hands-on experience in orthopaedics, geriatrics, neurology, oncology, general medicine, surgery, and paediatrics.
