Job Description

We are seeking an enthusiastic and dedicated Care Coordinator to join our team. As a Care Coordinator, you will be responsible for building strong relationships with clients and care workers, ensuring seamless communication and exceptional service.

Your primary responsibilities will include:

  • Managing a caseload of up to 35 clients living with various disabilities, coordinating their care plans and promoting independence.
  • Conducting home visits to introduce clients to our compassionate care workers and provide ongoing support.
  • Utilizing computer skills for efficient record-keeping and communication.

We offer a competitive salary based on experience, negotiable, along with 11.5% Superannuation, travel reimbursement for client visits, company-provided phone and laptop, flexible working arrangements, and a supportive team environment.

Requirements

To be successful in this role, you will need:

  • A minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker.
  • A valid QLD Driver's License.
  • A First Aid & CPR Certificate.
  • A Yellow Card/NDIS Worker Screening Clearance (paid).
