We are looking for a Clinical Assessor to join our team in Queensland, working with patients in both community and hospital settings. The successful candidate will be responsible for conducting thorough assessments of patients' physical, emotional, and social needs, and developing personalized care plans to address these needs and improve their quality of life.

Key Responsibilities:
  • Assess patients' physical, emotional, and social needs to develop effective care plans.

  • Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
  • Communicate effectively with patients, families, and healthcare professionals to achieve optimal outcomes.
Requirements:
  • Relevant tertiary qualification in a health-related discipline.
  • Minimum one year experience delivering services in aged care or to aged persons.
  • Valid drivers licence and endorsement to operate a motor vehicle.
  • Ability to travel to other facilities within the Aged Care Planning Regions as required.
