We are seeking a dedicated individual to join our team as a Disability Support Worker.

FlexiHome Care provides a wide range of in-home and community care support services to people with disabilities. Our goal is to empower our clients to live fulfilling lives, centered around their needs and preferences.

Main Responsibilities:

Person-Centred Care

Provide tailored support to help clients achieve their goals and aspirations.

Client Support Plans

Contribute to the development of personalized support plans that cater to each client's unique needs.

Support Services

Deliver a range of support services including personal care, living skills, transport, social activities, medication management, and other assistance as required.

Record Keeping

Maintain accurate and up-to-date records of client interactions and support provided.

Professional Relationships

Develop and maintain strong relationships with clients, families, caregivers, and key stakeholders.

Innovation and Improvement

Work collaboratively with the team to identify opportunities for improvement and implement innovative solutions to enhance the client experience.

Qualifications and Experience

Certificate III or IV in Disability, Individual Support, Aged Care, or Nursing is preferred. Relevant work experience in disability, aged care, or nursing is also highly valued.

Personal Attributes

The ideal candidate will possess excellent communication, problem-solving, and conflict resolution skills. They will be proactive, eager to learn, and able to work effectively in a fast-paced environment.

Requirements

This is a casual position working afternoons, Monday to Friday. The successful applicant will require a valid driver's license and access to their own vehicle.
