- Build strong, meaningful relationships with participants, their families, and support providers.

- C coordinate supports and negotiate service options that meet individual needs.
- Travel locally to meet with participants face-to-face, fostering a sense of connection and community.
- Balance time between participant-centered work and meeting performance targets.
To succeed in this role, you will need:
- Experience in disability services, community work, mental health, or related fields.
- Strong administration and organizational skills to manage multiple tasks and deadlines.
- Confidence in using digital tools and systems, particularly Excel, to track progress and data.
- The ability to build rapport, communicate effectively, and support people from diverse backgrounds.
