About the Role

We are seeking a skilled Case Manager to join our team at Mission Australia, working with young people who are experiencing or at risk of homelessness. As a Case Manager, you will provide case coordination, brief interventions, informal counselling, group facilitation, and education to support young people in achieving their goals.

Key Responsibilities
  • Conduct AOD assessments and intake processes for allocated young people
  • Develop and implement individualised AOD support plans, utilising SMART goals and referrals to supplementary services as needed
  • Collaborate with service staff to provide specialised support to address complex cases or emergency situations
  • Work late shifts, including three days a week until 8pm
  • Build networks with specialist AOD and Mental Health services, participating in service meetings
  • Provide brief intervention and informal counselling based on motivational interviewing techniques
  • Facilitate therapeutic and educational groups, covering AOD, mental health, and wellbeing
  • Provide AOD information and resources to young people, families, internal staff, external service providers, and community
Requirements for Success
  • Significant background in AOD and/or child, youth & family sectors
  • Confidence in case management and assessment processes
  • Demonstrated experience providing outreach, case management, case coordination, assessment, counselling, and crisis intervention skills
  • Experience working with at-risk young people, including knowledge of child and adolescent development and child and youth safe practices
  • Well-developed communication and interpersonal skills, including report writing and data collection
  • Full Driver Licence with no restrictions
