The delivery of a responsive mental health and social emotional well-being service requires the collaboration of a skilled team. As a Coach for Social Growth, you will engage in meaningful conversations to guide individuals towards services that align with their needs and strengths.
Your Key Responsibilities- You will listen attentively to each individual's unique story and provide informed referrals to connect them with the right services and supports.
- Maintain ongoing communication with service users and providers to ensure a seamless journey.
- Advocate personalized, strengths-based care and strive to make every service interaction as if it were for a friend or loved one.
- A commitment to serving others and innovating solutions.
- A deep understanding and empathy for every person's unique story.
- A strengths-based approach, focusing on what is strong, not just what is wrong.
- Degree in Social Work, Nursing, or other Allied Health Professions.
- NDISWS and WWCC.
- Driver's License.
- Act as a primary contact point, offering a warm, non-judgmental listening ear and expert guidance.
- Conduct comprehensive assessments to understand each individual's unique situation and needs.
