At War Memorial Hospital, our multidisciplinary rehabilitation facility offers coordinated care and discharge planning for older individuals. Our services include inpatient care, outpatient departments, and specialist community teams.

The Social Worker role sits within the Social Work Department and works closely with multidisciplinary teams to provide person-centred support to patients and their families. Key objectives include:

  • Providing high-quality support to inpatients and their families during stay
  • Planning and coordinating discharge services in conjunction with the multidisciplinary team
  • Working towards quality improvements in social work and multidisciplinary services in line with overall objectives

This full-time permanent position requires a minimum of 2 years' experience as a Social Worker, with demonstrated knowledge and expertise in social work practice in aged care and/or community settings.

Selection criteria include:

  • Minimum of 2 years' experience working as a Social Worker and a demonstrated knowledge and expertise in Social Work practice in an aged care and/or community setting
  • Demonstrated ability to provide effective Social Work services to older people, including skills in psychosocial assessment and management
  • Demonstrated ability to provide case management in consultation with the client, carer, and multidisciplinary team

Applicants must possess a Degree or postgraduate qualification in Social Work and be eligible for registration with the Australian Association of Social Workers (AASW).
