Job Summary

Myhomecare is seeking a skilled and compassionate Registered Nurse to join our team of healthcare professionals. As a Nurse Care Specialist, you will play a vital role in delivering high-quality patient care in the comfort of clients' homes.

Key Responsibilities

  • Conduct comprehensive assessments of clients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Provide expert clinical judgment and guidance to clients and their families, ensuring seamless transitions between care settings.
  • Collaborate with multidisciplinary teams to coordinate care, including medication management, wound care, and palliative support.
  • Monitor and report changes in clients' conditions, ensuring timely interventions and adjustments to care plans as needed.
  • Develop and maintain strong relationships with clients, families, and caregivers, fostering trust and open communication.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements, applying this knowledge to enhance client care and outcomes.

Requirements

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Minimum 2 years of experience in aged care, community health, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Current First Aid Certificate (HLTAID003/4) and CPR certification (HLTAID001).
  • The right to work in Australia and a current driver's license with comprehensive car insurance.
  • National Police Clearance and current NDIS Clearance Check.
