About the Role

We are seeking a highly skilled and experienced Clinical Care Coordinator to join our team at Prospero Health & Social Care. As a Clinical Care Coordinator, you will play a vital role in providing exceptional clinical support and coordinating services within our residential aged care facilities.

  • Conduct regular supervision sessions to assess team performance, identify areas for additional coaching or training, and document any incidents.
  • Manage customer relationships and address clinical aspects when required.
  • Lead clinical care teams to excellence, ensuring we meet and exceed industry standards.
  • Promote resident well-being and happiness, driving continuous improvement.
  • Collaborate with our healthcare partners to deliver high-quality care.

Requirements

  • Registered Nurse with AHPRA registration.
  • Proven track record in Aged Care leadership.
  • In-depth clinical knowledge.
  • Outstanding communication and leadership skills.
  • A genuine passion for senior care.
