The Role Reporting directly to the Operations Manager, you'll play a pivotal role by delivering high-quality care management services to our elderly clients in their own home environment.
You'll be more than just a case manager - you'll be an advocate, problem-solver, and partner in helping our clients navigate life's challenges with confidence and dignity.
Key Responsibilities:
• Design personalized care plans that prioritize independence, well-being, and dignity for each client.
• Collaborate closely with internal teams to ensure seamless delivery of high-quality care services.
• Establish and maintain strong relationships with clients, families, and stakeholders to foster trust and engagement.
• Identify potential risk factors and engage in co-management approaches to care planning, ensuring timely referrals to specialized services when necessary.
• Maintain accurate and up-to-date records in Sai's client care management systems.
Requirements:
• Proven experience in case management and aged care, with a passion for making a positive impact on people's lives.
• Strong analytical and problem-solving skills, with the ability to think creatively and outside the box.
• Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and colleagues alike.
• A valid driver's license and reliable vehicle are required for regular travel within the South-East region and Mornington Peninsula, with reimbursement provided for business-related expenses.
