Help Shape Lives as a Support Service Specialist

We are seeking skilled and passionate Case Workers to deliver support services to Australians in need.
About the RoleAs a Case Worker, you will provide trauma-informed case work delivering long-term and short-term support services. This includes school and/or home-based case management, needs assessments, practical assistance, referrals, and group-based interventions.
- Develop and implement individualized case plans and provide practical support.
- Facilitate referrals to internal and external services.
Your key responsibilities include:
- Maintaining accurate and timely case documentation.
- Working collaboratively with internal teams and external agencies to advocate for clients.
- SUPPORTING community development.
To be successful in this role, you will need:
- Relevant qualification or experience in the human services industry.
- Knowledge and experience working with client issues including domestic violence, substance abuse, and mental health.
- Excellent oral and written communication skills.
- A current NSW driver's license.
