About the Position

The Street to Home initiative provides assertive outreach services to engage individuals experiencing homelessness and rough sleeping. The program adopts a Housing First approach, prioritizing rapid access to stable, long-term housing. It integrates intensive case management and fosters collaboration with local partnerships to ensure a coordinated response that supports sustained housing stability and community reintegration.

Main Responsibilities:
  • Deliver mobile, outreach, and centre-based case management support to individuals and families facing vulnerabilities in Townsville.
  • Conduct comprehensive assessments and work with other agencies and organizations to source suitable support for clients.
  • Contribute to co-design processes and develop innovative ways of working to enhance the program's response to client needs.

Fundamental Qualifications:

  • A relevant degree or equivalent experience in social work, human services, or a related field.
  • Proven expertise in case management, preferably within the context of homelessness services.
