About this Opportunity:

The Street to Home program is a housing first initiative that aims to provide rapid access to stable, long-term housing for individuals experiencing rough sleeping and chronic homelessness.

In this role, you will be part of a team providing mobile, outreach and centre-based case management support to individuals and families experiencing vulnerabilities in Townsville.

Key Responsibilities Include:

  • Conducting holistic assessments and collaborating with other agencies to source appropriate support for clients.
  • Contributing to co-design processes and adapting ways of working to improve our response to client needs.

What You Bring:

  • Sound casework experience including holistic assessment of client needs and development of case plans.
  • Experience working with culturally and linguistically diverse backgrounds with respect and humility.
  • Well-developed interpersonal and written communication skills.
  • Strong computer literacy skills, including Microsoft office.
  • Effectiveness in contributing to interagency collaboration and ability to lead collaboration, advocate and network with internal and external stakeholders.

Mandatory Requirements:

  • Valid Working with Children Check.
  • Australian Driver Licence.
