Key Responsibilities- Provide advanced nursing care services to clients in the community, including those with complex health needs.
- Work collaboratively with clients, families, and other healthcare professionals to develop and implement individualized care plans.
- Ensure access and equity of services for all clients, particularly those from culturally and linguistically diverse backgrounds.
- Registered Nurse with at least 3 years of post-registration experience.
- Experience working with Aboriginal Clinical Health practitioners and supporting health outcomes for Aboriginal consumers.
- Excellent interpersonal and communication skills, with the ability to work effectively with individuals and families from diverse backgrounds.
