About the Opportunity

We are seeking a highly skilled and experienced Community Nurse to join our team at the Women's and Children's Health Network. As a Community Nurse, you will play a vital role in providing advanced nursing and quality client-centered care services to vulnerable young people and their families.

Key Responsibilities
  • Provide advanced nursing care services to clients in the community, including those with complex health needs.
  • Work collaboratively with clients, families, and other healthcare professionals to develop and implement individualized care plans.
  • Ensure access and equity of services for all clients, particularly those from culturally and linguistically diverse backgrounds.
Requirements
  • Registered Nurse with at least 3 years of post-registration experience.
  • Experience working with Aboriginal Clinical Health practitioners and supporting health outcomes for Aboriginal consumers.
  • Excellent interpersonal and communication skills, with the ability to work effectively with individuals and families from diverse backgrounds.
