Job Opportunity:

We are seeking a highly skilled and dedicated Registered Nurse to provide compassionate care to clients in their homes.

About the Role:

  • The successful candidate will conduct comprehensive clinical assessments, develop individualized care plans, and deliver high-quality nursing services to clients with complex health needs.
  • They will work collaboratively with other healthcare professionals to ensure seamless care coordination and achieve optimal client outcomes.
  • A key aspect of this role is educating clients and their families on self-care strategies, empowering them to live safely within their communities.

Your Key Responsibilities:

  • Conduct thorough clinical assessments and create personalized care plans that cater to the unique needs of each client.
  • Provide holistic nursing services, including medication management, wound care, diabetes management, and cognitive assessments, ensuring the best possible quality of life for our clients.
  • Develop strong relationships with clients and their families, fostering trust and understanding through empathetic communication.
  • Educate clients and their support networks on disease management, symptom control, and practical coping strategies, promoting independence and confidence.
  • Work closely with multidisciplinary teams to deliver coordinated care that addresses the physical, emotional, and social needs of our clients.

Requirements:

  • Current registration as a Registered Nurse with AHPRA
  • Availability to work flexible hours, including Tuesdays and Thursdays
  • Strong clinical knowledge and skills, with experience working with clients who have chronic and complex health needs
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and healthcare professionals
  • Capacity to work independently, prioritize tasks, and manage time effectively in a dynamic environment
