Healthcare Coordinator Position

This is a challenging and rewarding role that requires strong leadership skills, effective communication, and the ability to work in a fast-paced environment. As a Clinical Coordinator, you will be responsible for assessing, coordinating, and managing patient admissions.

The ideal candidate will have experience working in an acute or ambulatory/community setting, with a strong understanding of nursing care coordination models. They will also possess excellent problem-solving skills, be able to prioritize tasks effectively, and demonstrate a commitment to delivering high-quality patient care.

The role involves collaborating with multidisciplinary teams, specialist roles, and community organisations to ensure efficient and effective patient management and discharge planning. You will also be responsible for promoting the HITH service within the organisation and to external stakeholders, supporting efficiency and innovative ways of working with the EMR.

To be successful in this role, you will need to demonstrate strong autonomous decision-making skills, leadership abilities, and a willingness to take on new challenges. The role requires a Registered Nurse with current Victorian Driver's Licence, and experience within the acute and/or ambulatory/community setting is highly desirable.
