As a Support Coordinator, you'll work closely with our National Support Coordinator Manager to ensure that we can provide the best quality support coordination services possible to the community in Perth.

Key Responsibilities:

  • Work with participants to achieve their goals in line with their NDIS plan.
  • Provide ongoing support coordination services to participants in line with their NDIS plan.
  • Use a coaching approach to assist participants in capacity building.
  • Maintain accurate and up-to-date case notes.
  • Ensure Work Health and Safety guidelines are adhered to.
  • Engage with participants, collaborate with their supports and 3rd party organizations in a professional and collaborative manner.
  • Undertake work in line with our policies, procedures, and values.
  • Comply with all relevant legislation and code of conduct.
  • Maintain an up-to-date knowledge of the NDIS system and practices.
  • Maintain clear professional boundaries with participants.
  • Maintain an up-to-date knowledge of local services and organizations participants can link to.

Promoting Psychosocial Recovery

  • Assist participants to build capacity and resilience to live a full and contributing life.
  • Work collaboratively with participants, families, carers, and other services to identify, plan, design and coordinate NDIS supports.
  • Support clients to foster hope, identify and develop goals and work together towards achieving goals set out.
  • Build capacity to develop strong and respectful relationships.
  • Support to motivate the participants to have increased choice and control and increase their social and economic participation.
