Dedicated Care Professional

Delivering high-quality care management is a crucial aspect of our Home Care Package services in Toowoomba.
As a dedicated care professional, you will be responsible for building strong relationships with consumers and their family members to develop an effective support plan.
Key Responsibilities:
- Ensure timely delivery of packages while effectively managing consumer expectations
- Provide care and case management according to best practice standards, legislation, and organisational policies and procedures
- Utilise your skills and knowledge to deliver quality coordinated intervention to Home Care Package consumers
- Communicate effectively with a range of people, including older adults, staff, families, and particularly in relation to care coordination
Requirements:
- Relevant qualifications (Diploma level highly regarded)
- Excellent time management and organisational skills
- Extensive knowledge of Home Care Packages and community care services
- National Police Certificate
- Current First Aid and CPR Certificate
- Valid Driver's Licence and willingness to travel within the region
