About This Role:

We are seeking a skilled professional to join our team as a Community Engagement Specialist.

This role involves developing and maintaining relationships with clients, their families, external providers, and internal stakeholders. The ideal candidate will have previous experience in the health/aged care sector and possess high-level verbal and written communication skills.

The successful candidate will be responsible for assisting participants to maximize the benefits of their package/plan in an acceptable timeframe and within allocated resources. They will also ensure a smooth onboarding of new customers and facilitate ongoing support to our Aged Care & NDIS customers by providing an exceptional customer experience, engagement, and support in the achievement of individual goals.

Key responsibilities include capturing and maintaining client information across various management systems and engaging with internal and external stakeholders. The ability to work as part of a team and provide on-call support to stakeholders is essential.

Requirements:

  • Previous customer service experience in the health/aged care sector
  • High level verbal and written communication skills
  • Proven relationship management skills
  • Ability to work as part of a team and provide on-call support
  • Valid Australian Driver's Licence and NDIS Worker Screening Clearance/Criminal Record Check
  • Qualifications in Community Services, Human Services, Nursing, or equivalent
