Key Responsibilities

  • Work closely with NDIS participants to understand their goals, aspirations, and support needs
  • Develop and implement tailored support plans to help participants achieve their goals
  • Provide ongoing support and guidance to participants to ensure they have the skills and confidence to manage their supports
  • Collaborate with other professionals, such as healthcare providers and service providers, to ensure a holistic approach to support
  • Conduct regular reviews and assessments to ensure participants are achieving their goals and make adjustments to support plans as needed
