About this role:

We are seeking a full-time professional to deliver comprehensive care to patients with varying complexity on a fixed-term contract.
Your Key Responsibilities:
">- Collaborate with a multidisciplinary team to develop integrated service models and clinical protocols.
- Facilitate group sessions and health promotion activities as required.
- Provide timely assessments, referrals, accurate information, coordinated care, and follow-up.
- Facilitate discharge planning and advocate for patients' needs.
- Participate in clinical review meetings and continuous professional development.
About You:
">- You have a degree in Social Work with eligibility for membership with the Australian Association of Social Workers (AASW).
- Demonstrated experience in a sub-acute or similar healthcare setting.
- Strong communication, advocacy, and problem-solving skills.
- Excellent interpersonal skills with adaptability to evolving service needs.
- Proficiency in using Microsoft Office suite and other relevant documentation systems.
