Patient Care Specialist

We are looking for an experienced clinician to join our team as a Care Coordinator. In this role, you will play a vital part in ensuring that our clients receive comprehensive care and support in the comfort of their own homes.
About the Role
The ideal candidate will have a strong clinical background, preferably in an acute hospital setting, with excellent communication and interpersonal skills. You will work closely with our multidisciplinary team to provide high-quality patient care, while also focusing on building relationships with clients and their families.
Key Responsibilities:
- Conduct initial phone assessments, develop care plans, and set goals for clients
- Coordinate in-home services to support clients receiving care at home
- Maintain effective relationships with various stakeholders, including healthcare professionals and specialists
- Monitor patient progress and update care plans as necessary
- Liaise with healthcare professionals and specialists to ensure seamless care coordination
- Provide telephone support, education, and advice to clients and their families
Requirements:
- 3-5 years' experience in an acute hospital setting as a Registered Nurse with unrestricted registration
- Recent experience in care coordination (highly desirable)
- Strong customer service orientation and relationship management skills
- Excellent clinical reasoning skills
- Ability to work in a fast-paced environment
- Proficient computer skills to enable efficient data entry into patient electronic medical records
