Advanced Care Specialist Role

This senior clinical professional will provide expert support and assessment for residents, fostering a culture of continuous improvement in the Aged Care Home.

About the Position

The Advanced Care Specialist will be responsible for leading and mentoring care professionals to deliver exceptional clinical care. This role offers opportunities for specialization in areas such as palliative and dementia care while providing a high level of general clinical expertise.

Key Responsibilities:
  • Assessment and Planning: Conduct comprehensive clinical assessments, develop and implement tailored care plans, and provide guidance to the clinical team.
  • Clinical Leadership: Foster a collaborative environment, promoting evidence-based practice and staff development to enhance resident care.
  • Quality Improvement:
    • Promote best practices through education and training.
    • Support employees in delivering high-quality care by maintaining up-to-date knowledge and skills.
  • Documentation and Compliance:
    • Ensure accurate and timely completion of clinical documentation.
    • Participate in quality improvement initiatives to address emerging trends and issues.
  • Professional Development:
    • Maintain current professional practice and ongoing development.
    • Comply with regulatory standards and professional codes of conduct.
  • Workplace Safety:
    • Adhere to workplace health and safety policies and procedures.
    • Identify and report hazards, near misses, or incidents promptly.
Required Qualifications and Experience:

Essential Requirements:

  • Registered Nurse with a minimum of five years of experience.
  • Demonstrated leadership and mentorship skills in an Aged Care setting.
  • Postgraduate qualifications in a specialized clinical area related to aged care.
  • One year's experience working in the area of the postgraduate qualification or aged care.

Desirable Skills:

  • Three or more years' experience working in aged care.
  • Knowledge of adult learning principles and their application in clinical settings.
