As a Care and Support Coordinator with Ability Action Australia, you will work closely with our multidisciplinary team to evaluate, develop, and deliver evidence-based support plans for our clients. Your goal will be to ensure that our clients receive the highest level of care and support possible.
The ideal candidate will have current suitability as a Behaviour Support Practitioner at the Core or Proficient level, a degree-level qualification as an allied health professional, and excellent time management and organisational skills. You will also be proactive, motivated, and passionate about working in the disability sector.
