About the Role

The Care Coordinator will be responsible for delivering clinical care and governance of our wellbeing framework. The ideal candidate will foster a culture that is reflective of Bethel Disability Services' Vision, Purpose, and Values.

Key responsibilities include assessing, planning, implementing, and evaluating nursing care plans in collaboration with a multidisciplinary team to ensure holistic well-being of participants. The RN will also provide guidance and support to disability support workers to maintain high standards of care.

  • Conduct comprehensive health assessments of individuals with disabilities.
  • Develop, implement, and manage individualised care plans based on assessment findings.
  • Administer medications and treatments as prescribed, ensuring adherence to policies and procedures.

Clinical Care Objectives:

  • Ensure infection control practices are maintained.
  • Monitor participant's health status and response to interventions.
  • Collaborate with other healthcare professionals to provide coordinated care.
